1. BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF

Our fee for the basic services of the funeral director and staff include, but is not limited to, a proportionate share of the taxes, licenses, utilities and business expenses necessary to serve the public. Also included in this charge is the consultation with the family, clerical and administrative services, preparation and filing of necessary permits, consultation with the clergy, cemetery, crematory, common carrier, assistance with insurance papers, planning the funeral or memorial service and supervision of each service requested or required. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select.

2. EMBALMING

Embalming is a chemical process which provides temporary preservation of the body and eliminates health hazards. A person licensed by this state must complete this procedure. Except in certain special cases, embalming is not required by law. Embalming may be necessary however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial.

MERCHANDISE

We offer many different styles and prices of caskets, vaults and an alternative container from which to select. Since many caskets appear similar, they may differ greatly in quality and construction, we offer the following in order to assist you in making an informed decision.

Protective
These caskets are designed by the manufacturer to resist the entrance of air, water and other outside elements. They may be constructed of varying gauges of steel, copper or bronze.

Non-Protective
These caskets are not designed by the manufacturer to resist the entrance of air, water or other outside elements. They may be constructed of metal, hardwood, or wood products covered with fabric.

Disclaimer of Warranty
The only warranty of the casket or any merchandise sold in connection with this service is the express written warranty, if any, granted by the manufacturer. This funeral home makes no warranty, expressed or implied, including an implied warranty of merchantability and an implied warranty of fitness for a particular purpose, with the respect to the casket to the casket and/or outer burial container.

8. FORWARDING REMAINS TO ANOTHER FUNERAL HOME

This charge includes removal of remains, basic services of staff, embalming, obtaining and filing necessary paperwork. This charge does not include any rites or ceremonies, merchandise such as casket, air tray or combination shipping unit.

This charge includes basic services and transfer of remains to funeral home, cemetery or crematory. This service does not include any rites or ceremonies or merchandise such as casket, outer burial container and any cemetery, crematory or church fees.

10. IMMEDIATE BURIAL

This charge includes basic services of the staff, removal of remains from place of death, sanity care of the remains and transportation of the deceased to the cemetery for burial. This charge does not include any rite or ceremonies, embalming or other preparations of the deceased, casket, outer burial container and cemetery fees.

11. DIRECT CREMATION

This charge includes basic services of the staff, obtaining and filing necessary paperwork, removal of the deceased from the place of death, sanitary care of the remains, transportation of the deceased to the crematory, cremation container, cremation fee and the medical examiner's fee. This charge does not include any rites or ceremonies, urn, obituary notice, urn vault or cemetery fees.

If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials. The container we provide, which is required by state law, has a wooden base and a cardboard top.

ACCEPTABLE FORMS OF PAYMENT ARE ANY APPLICABLE GOVERNMENT FORMS, CASH, CREDIT, BANK CHECK OR PERSONAL CHECK. WE APOLOGIZE IN ADVANCE FOR ANY INCONVENIENCE THIS MAY CAUSE. SHOULD THE COMPLETE BALANCE NOT BE PAID AT THE TIME OF SERVICE, ALL CASH ADVANCE ITEMS ARE REQUIRED TO BE PAID. THESE ITEMS INCLUDE BUT ARE NOT LIMITED TO, CEMETERY FEES, CHURCH FEES, CERTIFIED COPIES OF THE DEATH CERTIFICATE, AND ANY OBITUARY NOTICES.
